Employment Requirements

Sheriff’s Card Referral Form
Requirement for Prospective Entertainers. The referral form is picked up at the club by the prospective entertainer at the time of a successful audition.The form must be filled-out in black ink, and all prospective entertainers age 25 or younger must bring a CERTIFIED copy of their birth certificate when they apply for their Sheriff’s Card.

Sheriff’s Card (Work Card)
Requirement of all club positions. Entertainers, Waitresses, Bartenders, Shot Girls, Floor Hosts, and Managers.

Nevada Business License
Requirement of Entertainers and Shot Girls.

TAM Card (Techniques of Alcohol Management)
Requirement of positions that involve handling alcohol. Waitresses, Bartenders, Shot Girls, Floor Hosts, and Managers.

Health Card
Requirement of positions that involve handling alcohol. Waitresses, Bartenders, Shot Girls, Floor Hosts, and Managers.

Sheriff’s Card Eligibility and Issuance

To apply for a Sheriff’s Card, applicants must:

  • Have a signed Sheriff’s Card Referral Form from your prospective employer.
  • Be 21 years old if dispensing or selling alcoholic beverages.
  • Certified copy of birth certificate must be shown to Sheriff’s Department by applicants 25 years of age and younger. No exceptions.
  • Persons not born in the U.S. or U.S. Territory must show one of the following: Naturalization Certificate; U.S. Passport, U.S. birth certificate; and/or Alien Card. This includes persons of U.S. Citizenship born abroad.
  • Pay the appropriate fees.
  • You must show two forms of acceptable I.D. (any state-issued driver’s license; state-issued I.D.; military I.D.; DD214; U.S. Passport; Social Security Card; Naturalization Certificate; Baptismal Certificate, and Alien I.D. Card.

Work card processing includes interviewing, fingerprinting and photographing. The work card is similar in size to a driver’s license and is issued the same day as applied for provided there are no complications during the process.

The LVMPD issues work cards at two locations:

Las Vegas office

Work cards currently are issued from 8 a.m. to 4 p.m. Monday-Friday and closed all federally recognized holidays.
